Do not splice incompatible measures into one trend line
If a goal begins with instructional-level passages and later switches to grade-level passages, or if prompted performance is replaced with independent performance, annotate the change and avoid treating the points as directly interchangeable. A cleaner option is often to start a new series while retaining the old one for context.
The same rule applies after a scoring change. If staff redefine an “independent step,” update the data system prospectively and document the date. Retroactively rescoring old observations from memory can create a cleaner graph but a weaker record.
How to get a baseline
A progress-monitoring graph needs a defensible starting point. Collect baseline data with the same probe, scoring rule, support level, and unit that will be used after instruction begins. For variable skills, three comparable points are usually more useful than a single “best” or “worst” performance.
Choose the tool from the goal, not the other way around. WCPM fits oral-reading fluency; percent correct across defined items may fit computation; latency fits initiation; frequency or duration may fit some behaviors; task-analysis percent independent fits multistep routines; an anchored rubric can fit written products.
Write the baseline date and condition on the graph. If the student receives a new support, schedule change, or substantially different task demand, mark that event instead of pretending the data series stayed identical.

The monitoring examples below illustrate measurement systems rather than ready-to-copy goals; the student’s actual goal and target must be individualized from current baseline data.

Aim lines, trend lines, and instructional decisions
An aim line is the expected path from baseline to the annual criterion. A trend line summarizes the direction of the student’s actual data. You do not need sophisticated software to use the idea: a simple graph can show whether performance is generally closing the gap, staying flat, or becoming more variable.
Decision rules should trigger review, not automatic goal changes. A common classroom rule is to review instruction when three or four consecutive points fall below the aim line. Before acting, check probe comparability, attendance, instructional dose, fidelity, and whether the student has the prerequisite skill. If the trend remains weak after an instructional adjustment, document the change and continue collecting the same measure so the team can evaluate the response.
Use decision rules as prompts for review, not automatic verdicts
Teams sometimes use rules such as “three or four points below the aim line means change instruction.” That can be a useful local convention when data are frequent and comparable, but it is not an IDEA mandate and should not override the quality of the data. A single difficult probe, student illness, changed prompt level, or different task form can create a false signal.
When several points fall below the expected path, review measurement fidelity, implementation, instructional match, attendance, and prerequisite skills. If those are sound, change the instructional plan and mark the change on the graph. Continue collecting comparable data long enough to see whether the new approach changes the trend before deciding that the annual criterion itself is the problem.
How to write the matching present level
The PLAAFP should supply the graph’s starting value and explain why that skill matters educationally. Example: “Across three paragraph-writing samples scored with the same four-point organization rubric, Alexis earned 1, 2, and 1; she currently needs adult conferencing to sequence ideas before drafting.” The annual goal can then use the same rubric and define the expected level of independence.
Do not replace a baseline with a later score when the student improves. Preserve the original value so the team can see growth from the point at which the IEP began.
Progress monitoring
Write the monitoring plan before the first data point is collected: tool, frequency, scorer, setting, and review rule. Classroom data may be gathered more often than family progress reports. Under 34 CFR §300.320(a)(3), the IEP must state how progress toward annual goals will be measured and when periodic reports will be provided.
Use an aim line to show the path from baseline to the annual target and a trend line to summarize actual performance. Do not apply a rigid “three points” or “four points” rule when probes are not comparable. A cluster of points below the aim line is a reason to examine instruction, dosage, attendance, prompt dependence, and measurement fidelity—not automatic proof that the annual goal was written incorrectly.
Document instructional changes on the graph or in a linked note. That turns progress monitoring into a decision tool: the team can see not only whether the student improved, but what changed before the improvement or plateau.
Common mistakes
Watch for these drafting traps:
Collecting data only right before report cards. Changing probe difficulty without annotation. Using classroom grades instead of the goal's metric. Graphing prompted and independent performance together. Writing comments that contradict the data. Changing the goal before checking implementation.
Audit the graph and the raw data together. The dates, probe conditions, units, phase changes, and instructional adjustments should line up, and the decision rule should be understandable without reverse-engineering a spreadsheet. A clean graph cannot rescue inconsistent underlying measures.
